Make an appointment with a Doctor near you : Approved Physician List

Discuss your medical history with your MD or DO and identify which qualifying medical conditions will allow you to get your meical marijuana card.

Qualifying Medical Conditions
  • Debilitating medical conditions*
  • Cancer
  • Epilepsy
  • Glaucoma
  • Crohn’s disease
  • HIV or AIDS
  • Cachexia
  • Persistant muscle spasms
  • Seizures
  • Severe nausea
  • Severe pain

Debilitating medical conditions include:

  • Post Traumatic Stress Disorder (PTSD)
  • An Autism Spectrum Disorder
  • Any condition for which a physician could prescribe an opiod.

Step 2) Residency Requirements

Understand the Residency Requirements

Adults

To be eligible for a medical marijuana card you must:
Be a Colorado resident
Be 18 or older
Have a qualifying medical condition

Minors

Minors
Minors are eligible for a medical marijuana card if:
Minor and the primary parent are Colorado residents
Minor has a qualifying medical condition​

All minor patients must see two providers and submit two provider certifications.

Step 3) Create A Profile

Create Your Profile with the Department of Health

Register with DHSS Online

Visit the Department of Health: health.gov to create a profile.  You’ll need to be able to prove residency in the state of Missouri. 

Step 4) Apply For Your Card
